Presumptive Drug Tests: Cutoffs, False Positives, and Confirmation

A presumptive drug test is a fast, inexpensive screening test that checks a urine, saliva, hair, or seized-substance sample for the possible presence of drugs. It gives a result in minutes, but it does not give a final answer. A positive on a presumptive screen only means the sample needs a second, more precise laboratory test to confirm what — if anything — is actually there.

That two-step design is the whole point. Screening is cheap enough to run on everyone; confirmation is expensive enough that you only want to pay for it when the screen flags something.

How a Presumptive Test Works

Most presumptive tests for people rely on immunoassay technology. The test uses antibodies designed to bind to specific drug molecules or their metabolites. When the target substance is present above a set cutoff concentration, the antibody reaction produces a visible signal — usually a colored line on a strip or a measurable change on a lab instrument. A single sample can be screened for several drug classes at once, with results in minutes.

Law enforcement uses a different tool for identifying unknown substances found in the field. Colorimetric reagent kits contain chemicals that change color when they meet a particular drug class. An officer places a small amount of the material into the pouch and reads the color. These kits are cheap and portable, and they are also the least reliable form of presumptive testing. Household substances have been known to produce the same color reactions as illegal drugs, so anything a field kit identifies should be sent to a forensic laboratory before it is relied on in court.

Sample Types and Detection Windows

The sample used shapes what the test can find and how far back it looks.

  • Urine is the most common sample for workplace and probation testing. Drugs are generally detectable for one to seven days after use, though chronic heavy use can extend that window considerably.
  • Oral fluid is collected by swab under direct observation, which makes tampering very difficult. It detects recent use, typically within five to 48 hours, so it is better suited to post-accident or reasonable-suspicion testing where the question is whether someone is impaired now.
  • Hair captures a longer history. A 1.5-inch sample covers roughly 90 days of exposure. It is less useful for detecting recent use, since drugs take about a week to become incorporated into the hair shaft.

What Drugs a Presumptive Test Screens For

The traditional workplace screen covers five drug classes: amphetamines, marijuana (THC), cocaine, opiates, and phencyclidine (PCP). A 10-panel test adds barbiturates, benzodiazepines, methadone, methaqualone, and propoxyphene.

The federal government expanded its standard panel effective July 2025. Federal workplace tests now also screen for fentanyl and norfentanyl, hydrocodone, hydromorphone, oxycodone, oxymorphone, and MDMA/MDA.2Federal Register. Cutoff Levels and What “Negative” Really Means

Every presumptive test has a cutoff concentration, measured in nanograms per milliliter. If a drug or metabolite is present below that threshold, the test reports negative even though trace amounts exist. This is deliberate. A floor reduces false positives from incidental environmental exposure and keeps the test focused on meaningful drug use.

For the federal urine panel, some key initial screening cutoffs are 50 ng/mL for the marijuana metabolite, 150 ng/mL for the cocaine metabolite, 500 ng/mL for amphetamines, 2,000 ng/mL for codeine and morphine, and 1 ng/mL for fentanyl.2Federal Register. The marijuana screen cutoff for oral fluid is just 4 ng/mL, compared to 50 ng/mL for urine.

These numbers matter if you are trying to make sense of a result. A negative does not necessarily mean zero drugs in your system. It means whatever was present fell below the line the test was designed to catch.

Why False Positives Happen

Immunoassay tests detect molecular shapes, and some legal substances have shapes similar enough to trigger the reaction. This is called cross-reactivity, and it is the most common source of false positives. If you tested positive on a presumptive screen and have not used any illegal drugs, one of these may be responsible:

  • Dextromethorphan, the active ingredient in Robitussin, Delsym, and other cough suppressants, can trigger a false positive for PCP.
  • Diphenhydramine (Benadryl) has been reported to cause false positives for opioids, though typical doses may not reach the threshold.
  • Poppy seeds contain trace amounts of codeine and morphine. Drug tests are sensitive enough to detect these traces even though the quantities are far too small to cause intoxication. The opiate content varies by growing conditions and processing, so there is no reliable “safe amount” to eat before a test.   • NSAIDs like ibuprofen occasionally produced false positives for cannabinoids on older immunoassay formulations, though modern assays have largely eliminated this problem. False negatives happen too, usually when drug concentration falls below the cutoff or when someone has tampered with the sample. Laboratories run validity checks to catch common forms of adulteration like dilution, chemical additives, or substitution with someone else’s urine.

What Happens After a Presumptive Positive

A presumptive positive is not a final answer. It is the start of a verification process, and the steps that follow depend on the setting.

Laboratory Confirmation

The gold standard for confirmation is gas chromatography-mass spectrometry (GC-MS) or liquid chromatography-mass spectrometry (LC-MS/MS). These instruments physically separate the molecules in the sample and identify each one by its unique mass signature, which eliminates the cross-reactivity problem that trips up immunoassays.5National Center for Biotechnology Information. In federally regulated workplace programs, confirmatory testing is mandatory before any result is reported as positive. Private employers are not always required to confirm, and the rules vary by state. If an employer takes action based on a screening result alone, that result was never verified by a method that can distinguish cold medicine from methamphetamine.

Medical Review Officer

For DOT-regulated testing and many other workplace programs, a confirmed positive goes to a Medical Review Officer before it reaches the employer. The MRO is a licensed physician acting as an independent gatekeeper. Before reporting a result as positive, the MRO contacts you to ask whether you have a legitimate medical explanation, such as a valid prescription for the detected substance.6U.S. Department of Transportation. Medical Review Officers If you can document a prescription, the MRO reports the result as negative. This step catches the most obvious false positives before they ever affect your job.

Consequences of a Verified Positive

Once a result is both confirmed by the laboratory and verified by the MRO, consequences follow. A DOT safety-sensitive employee must be immediately removed from safety-sensitive duties on a verified positive.7U.S. Department of Transportation. In clinical settings, a confirmed positive guides treatment decisions rather than penalties.

Where Presumptive Tests Are Used

Pre-employment and random workplace drug tests almost always start with a presumptive screen. For safety-sensitive jobs regulated by the Department of Transportation, federal rules spell out which drugs to screen for, what cutoff concentrations to use, and what happens when a result comes back positive.2Federal Register. Law enforcement uses presumptive testing in two distinct ways: roadside oral fluid devices to screen drivers suspected of drug impairment,8National Center for Biotechnology Information. Courts impose drug testing as a condition of probation and supervised release to deter use and support treatment.9United States Courts. Limitations Worth Knowing

A positive result does not prove impairment. Urine tests in particular detect metabolites that linger long after any psychoactive effect has worn off. Someone who used marijuana days ago will test positive on a urine screen even though they are sober at the time of collection. Oral fluid testing narrows that gap by focusing on a shorter detection window, but no presumptive test measures impairment the way a blood alcohol test measures intoxication.

Cost is a large part of why presumptive tests exist at all. A standard urine or saliva screen typically runs between $20 and $40, while confirmatory GC-MS or LC-MS/MS testing costs substantially more. Screening cheaply and confirming selectively is what makes large-scale testing possible.
