Does Medicare Cover Sildenafil for ED or PAH?

Medicare covers sildenafil only when it is prescribed for pulmonary arterial hypertension, a serious lung condition. Why ED Prescriptions Are Excluded

This is not a plan-by-plan decision. Congress amended the Social Security Act in 2005 to exclude any drug “used for the treatment of sexual or erectile dysfunction” from the definition of a covered Part D drug. When Sildenafil Is Covered: Pulmonary Arterial Hypertension

Sildenafil is FDA-approved under the brand name Revatio for treating pulmonary arterial hypertension, a progressive disease involving dangerously high blood pressure in the lung arteries.3Food and Drug Administration. Coverage still depends on the specific plan. Each Part D plan maintains its own formulary, and not every one includes sildenafil for PAH at the same tier or with the same cost-sharing. Before enrolling in a plan or filling a prescription, look up Revatio or generic sildenafil on the plan’s formulary to confirm it is listed and check the tier.

Prior Authorization and Step Therapy

Even when a plan covers sildenafil for PAH, it often adds requirements before it will pay. Prior authorization is the most common: your doctor submits documentation showing the prescription is medically necessary for PAH, and the plan must approve the request before the pharmacy dispenses the drug under your Part D benefit.4Medicare. If Your Plan Denies Coverage

You, your representative, or your prescriber can file a formulary exception request asking the plan to cover a drug that isn’t on the formulary, or to waive a requirement like prior authorization or step therapy. The plan must respond within 72 hours for a standard request. What You’ll Pay in 2026

If your plan covers sildenafil for PAH, your costs follow the standard Part D structure. The maximum annual deductible in 2026 is $615, though many plans set it lower or waive it.8Medicare. Once your out-of-pocket spending reaches $2,100 for the year, you enter catastrophic coverage and pay nothing for covered Part D drugs for the rest of the calendar year.9CMS. Spreading Out the Cost

Since 2025, the Medicare Prescription Payment Plan has let you spread out-of-pocket drug costs into monthly installments instead of paying the full amount at the pharmacy counter. Anyone with Part D or a Medicare Advantage plan that includes drug coverage can opt in at any time during the year. There is no fee and no interest, even if you pay late.10Medicare.gov. Your monthly bill equals what you currently owe plus any previous balance, divided by the number of months left in the year. Total payments through the program in 2026 can never exceed $2,100. Extra Help for Limited Incomes

If your income and savings are limited, the Medicare Extra Help program (also called the Low-Income Subsidy) can reduce your Part D premiums, deductibles, and copayments. For 2026, you may qualify with annual income below $23,940 for an individual or $32,460 for a married couple, and countable resources under $18,090 for an individual or $36,100 for a couple.11Medicare. Extra Help applies to covered Part D drugs. It will lower your costs for sildenafil prescribed for PAH, but it does not create coverage for sildenafil prescribed for ED, which stays excluded under federal law regardless of income.

Paying for Sildenafil for ED

Most Medicare beneficiaries who want sildenafil for erectile dysfunction pay cash. Generic sildenafil has been on the U.S. market since the FDA approved the first generic version in 2016, and competition among manufacturers has driven prices well below what brand-name Viagra once cost. Prices vary widely by pharmacy, so shopping around or using a pharmacy discount program can make a real difference.

Pfizer offers a Viagra savings card, but the fine print excludes anyone whose prescription is reimbursed by Medicare, Medicaid, TRICARE, or any other federal or state health program.
