Does a Drug Test Detect Alcohol? Tests, Windows, and EtG Cutoffs

A standard drug test does not detect alcohol. The 5-panel and 10-panel screens used for most employment, probation, and medical screening are built around controlled substances, and alcohol is not one of the analytes they look for. If someone needs to know whether you have been drinking, they have to order a separate alcohol test.

That distinction matters, because “drug and alcohol testing” often gets spoken as one phrase even though the lab runs two different tests. Knowing which one you are actually being given tells you what will show up and what won’t.

What Standard Drug Panels Cover

The most widely used workplace drug test in the United States is the 5-panel urine screen. Under the federal Mandatory Guidelines for Federal Workplace Drug Testing Programs, that panel covers marijuana, cocaine, opioids (including codeine, morphine, oxycodone, and hydrocodone), amphetamines (including methamphetamine and MDMA), and phencyclidine (PCP).1Federal Register. Employers who want more coverage can order expanded panels. A 10-panel test typically adds barbiturates, benzodiazepines, methadone, and methaqualone to the standard five.3Quest Diagnostics. Alcohol is not on any of them, and the federal guidelines have never authorized it as an analyte.1Federal Register. When Alcohol Testing Gets Ordered Alongside a Drug Test

DOT Safety-Sensitive Jobs

The main exception to the “no alcohol on drug tests” rule is the Department of Transportation. Under the Omnibus Transportation Employee Testing Act of 1991, every DOT agency must run alcohol testing for safety-sensitive employees, separate from the drug panel.4Federal Motor Carrier Safety Administration. DOT alcohol tests can be run after qualifying accidents, on reasonable suspicion, at random, before an employee returns to duty after a violation, and during follow-up monitoring. Pre-employment alcohol testing is allowed but optional; if an employer chooses to do it, it must be offered to every applicant, not selectively.5US Department of Transportation. Private Employers, Courts, and Probation

Outside DOT-regulated industries, a private employer can add alcohol to its testing protocol at its own discretion. This is common in construction, healthcare, manufacturing, and return-to-work programs after a substance abuse issue. If a private employer tells you to take a “drug test,” that test might quietly include alcohol, so ask what the panel actually covers rather than assuming.

Courts and probation officers routinely order alcohol-specific testing, particularly after DUI convictions and in custody disputes and sobriety compliance programs. These programs often use the more sensitive biomarker tests described below, because they need to catch drinking that happened days or weeks earlier, not just that morning.

The Tests That Actually Detect Alcohol

Breath

Breathalyzers estimate blood alcohol concentration by measuring ethanol in deep lung air as you exhale.7PubMed Central. Blood

A blood draw is the most direct measurement of ethanol in the bloodstream and is treated as the gold standard for accuracy. Blood tests can also check for two long-window biomarkers. Urine

A basic ethanol urine test just checks whether alcohol is still in the system, which only works within a narrow window. The more useful version looks for ethyl glucuronide (EtG) and ethyl sulfate (EtS), two metabolites the liver produces when processing alcohol. EtG and EtS stay detectable in urine well after the alcohol itself has been eliminated.11Quest Diagnostics. Hair

Hair testing reveals drinking patterns over months rather than days. Scalp hair grows at roughly one centimeter per month, so a standard 3-centimeter segment captures about three months of history. Labs analyze the hair for EtG, which reaches the hair through sweat, and for fatty acid ethyl esters (FAEEs), which are incorporated through sebum.12ScienceDirect. Transdermal (SCRAM) Bracelets

Courts sometimes order continuous alcohol monitoring through an ankle bracelet, most often the SCRAM CAM device. About 1% of consumed alcohol exits the body through the skin as insensible perspiration. How Long Alcohol Shows Up on Each Test

Detection windows vary dramatically by test type. The difference between a breath test and a hair test can be the difference between a few hours and a few months.

EtG Cutoffs and False Positives

If you are subject to EtG urine testing, the cutoff threshold the lab uses changes what counts as a positive. A 500 ng/mL cutoff reduces the false positive rate to roughly 3% but may miss lighter drinking more than a day earlier. A 100 ng/mL cutoff catches about 79% of heavy drinking over five days, but false positives climb to around 16%. Courts and treatment programs that want to catch any drinking tend to use the lower threshold; employment testing leans higher.

The sensitivity cuts both ways. SAMHSA has issued advisory guidance recommending caution when interpreting positive EtG results, because everyday products can produce enough ethanol exposure to trigger one.17PubMed Central. Hand sanitizer can lead to trace absorption through the skin or inhalation, especially with heavy healthcare use. Some cough syrups and cold medications also contain ethanol. Fermented foods add another layer: kombucha, sauerkraut, soy sauce, and overripe bananas all contain or trigger small amounts of ethanol. At 500 ng/mL these are unlikely to cause a positive; at 100 ng/mL the risk is real.

Breath tests have their own false-positive problem, but a shorter one. Using mouthwash or a breath spray with alcohol immediately before a breath test can produce a falsely elevated reading, which is why DOT procedures and most law enforcement protocols require a waiting period before testing.

If you are on a monitoring program that uses EtG, the safest approach is to switch to alcohol-free mouthwash, avoid hand sanitizer when you can, and read the labels on any over-the-counter medications you take. A false positive is a headache to unwind even when the explanation is legitimate.
